Activity, Sleep and Nutrition Tracking Service

Available on G-Cloud 10

The Activity, Sleep and Nutrition Tracking Service allows patients to securely share activity, sleep and diet and nutrition data gathered from readily available consumer apps and wearable devices. The data can be used by health professionals to deliver a digitally enabled health and well-being service to patients with a range of health conditions.

How it works

  1. Patient Onboarding

Patients with long terms conditions will be onboarded to the service by health professionals. Baseline clinical data will be established. Wearables and connected hardware will be supplied.

  2. Data Gathering

Patients gather health data from the wearables and connected hardware supplied during onboarding. The health data is automatically and securely shared with the Lenus Digital Health and Care Platform.

  3. Clinical Dashboard

Patient data for activity, sleep and nutrition will become available in the clinical dashboard, providing health professionals with a dedicated interface that displays and aggregates health data across all onboarded patients.

  4. Machine Learning

Data rules help trained health professionals to identify patterns and insights that will trigger personalised interventions or escalation.

  5. EHR Integration and Ongoing Communication

Through the Lenus Digital Health and Care Platform, healthcare professionals can access the patient’s Electronic Health Record (EHR) and augment it with data from the Activity, Sleep and Nutrition Tracking service. Messages can be sent to patients (or their designated care team) via the Lenus Digital Health and Care Platform. Diagnostics can also be created to gather patient reported outcomes and qualitative insights from patients.

The Opportunity

Long term conditions like diabetes, obesity, angina and chronic respiratory problems can significantly impact physical activity, sleep and nutrition.

The Activity, Sleep and Nutrition Tracking Service seeks to deliver improved outcomes for long term conditions by driving behavioural change, empowering patients to be more active whilst also supporting better monitoring and reporting of activity and sleep levels as well as nutrition tracking.

The Data

The connected hardware and apps provide the following data:

  • Daily activity
  • Calories
  • Distance travelled
  • Progress against target
  • Sleep
