Care Plan creation
The care plan is authored by the health professional with the patient using the service. The care plan is shared electronically with the patient or their nominated representative.
Care teams and nominated health professionals can request access to view the patient care plan. Patients can authorise who and what has access to their care plan.
Care plans can be made available for review within clinical dashboards or EHRs as part of new care models.
New versions of care plans can be created with consent of patient with access to previously created care plans with complete audit of changes.
EHR Integration and Ongoing Communication
Through the Lenus Digital Health and Care Platform, health and care professionals can access the patient’s Electronic Health Record (EHR) and Care Plan. Messages can be sent to patients (or their designated care team) via the Lenus Digital Health and Care Platform. Diagnostics can also be created to gather qualitative insights from patients too.
The Care and Support Plan Service is delivered on a cost per patient basis for a minimum period of 12 months. Exact costs depend on total number of patients, duration of service and required integrations with existing systems.
A licence for the Lenus Digital Health and Care Platform will be required to enable the service. Your organisation may already have a licence. Contact us to discuss. Learn more about licencing and procurement.
