Diabetes Prevention Service

Available on G-Cloud 10

The Diabetes Prevention Service allows patients who are prediabetic to securely share activity, sleep and nutrition data, gathered from apps and wearables. Insight gleaned from the data can be used by health professionals to support patients in preventing prediabetes developing in to Type 2 diabetes.

How it works

  1. Patient Onboarding

Patients who are prediabetic will be onboarded to the service by health professionals. Baseline physiology data is established and initial weight and activity targets set. Wearables and connected hardware will be supplied.

  2. Data Gathering

Patients gather activity, diet and nutrition and weight data from the apps, wearables and connected hardware supplied during onboarding. The health data is automatically and securely shared with the Lenus Digital Health and Care Platform.

  3. Clinical Dashboard

Patient data will become available in the clinical dashboard, providing health professionals with a dedicated interface that displays and aggregates health data from their patients.

  4. Machine Learning

Data rules help trained health professionals to identify patterns and insights that will trigger personalised interventions or escalation, in line with defined clinical protocols.

  5. Providing Interventions

Rules will help identify those patients at high risk of lapse and support guided interventions as required.

  6. EHR Integration and Ongoing Communication

Through the Lenus Digital Health and Care Platform, healthcare professionals can access the patient’s Electronic Health Record (EHR) and augment it with data from the Diabetes Prevention service. Messages can be sent to patients (or their designated care team) via the Lenus Digital Health and Care Platform. Diagnostics can also be created to gather qualitative insights from patients too.

The Opportunity

Prediabetes is a condition that is linked to obesity and is growing in prevalence across the UK and around the world.

It is estimated that as many as 7 million people in the UK have prediabetes and are, therefore, at high risk of Type 2 diabetes.

1 in 3 of the adult people estimated to have prediabetes
10% weight loss cuts the chance of developing type 2 diabetes in half
30mins brisk walk per day can reduce risk by 30%

The Data

The connected hardware and apps provide the following data:

  • Daily activity
  • Calories
  • Distance travelled
  • Progress against target
  • Sleep
