How it works
Patients at risk of falls will be onboarded to the service by health and care professionals. Baseline clinical data will be established. Wearables and connected hardware will be supplied.
Patients actively and passively gather health. movement and mobility data from the wearables and connected hardware supplied during onboarding. The data is automatically and securely shared with the Lenus Digital Health and Care Platform.
Patient data will become available in the clinical dashboard, providing health and care professionals with a dedicated interface that displays and aggregates health, movement and mobility data across all onboarded patients.
Data rules help trained health and care professionals to identify patterns and insights that will trigger personalised interventions or escalation.
Trained algorithms will help to determine if a risk of a fall is heightened. This will guide if proactive health or social care interventions are required.
EHR Integration and Ongoing Communication
Through the Lenus Digital Health and Care Platform, health and care professionals can access the patient’s Electronic Health Record (EHR) and augment it with data from the Falls Prevention service. Messages can be sent to patients (or their designated care team) via the Lenus Digital Health and Care Platform. Diagnostics can also be created to gather qualitative insights from patients too.
The Falls Prevention Service is delivered on a cost per patient basis for a minimum period of 12 months. Exact costs depend on total number of patients, duration of service and required integrations with existing systems.
The cost per patient will be significantly less than the cost of an emergency hospital admission.
A licence for the Lenus Digital Health and Care Platform will be required to enable the service. Your organisation may already have a licence. Contact us to discuss. Learn more about licencing and procurement.
