Hypertension Management Service

Available on G-Cloud 10

The Hypertension Management Service allows patients to log regular blood pressure and pulse readings taken by a cuff (connected hardware) and securely share this data with health professionals, thereby reducing the number of routine readings needing to be taken in acute settings.

How it works

  1. Patient Onboarding

    Patients who exhibit hypertension are onboarded to the service by health professionals. Baseline clinical data will be established. Wearables and connected hardware will be supplied.

  2. Data Gathering

    Patients gather blood pressure data from the wearables and connected hardware supplied during onboarding. The blood pressure data is automatically and securely shared with the Lenus Digital Health and Care Platform.

  3. Clinical Dashboard

    Patient data will become available in the hypertension clinical dashboard, providing health professionals with a dedicated interface that displays and aggregates health data across all onboarded patients.

  4. Machine Learning

    Data rules help trained health professionals to identify patterns and insights that will trigger personalised interventions or escalation, in line with defined clinical protocols.

  5. Providing Interventions

    Trained algorithms will help to determine if blood pressure readings indicate risk. This will guide if proactive health or social care interventions are required

  6. EHR Integration and Ongoing Communication

    Through the Lenus Digital Health and Care Platform, healthcare professionals can access the patient’s Electronic Health Record (EHR) and augment it with data from the Hypertension Management service. Messages can be sent to patients (or their designated care team) via the Lenus Digital Health and Care Platform. Diagnostics can also be created to gather qualitative insights from patients too.

The Opportunity

High blood pressure affects more than 1 in 4 adults in England, with people from the most deprived areas 30% more likely than the least-deprived to have high blood pressure. Over 2.1 million people under the age of 45 had high blood pressure in England in 2015.

It's the 3rd biggest risk factor for premature death and disability in England after smoking and poor diet.

At least half of all heart attacks and strokes are associated with high blood pressure and it's a major risk factor for chronic kidney disease, heart failure and dementia.

The Costs

The Hypertension Management Service is delivered on a cost per patient basis for a minimum period of 12 months. Exact costs depend on total number of patients, duration of service and required integrations with existing systems.
