The Hypertension Management Service uses connected devices to allow patients to share blood pressure and pulse readings with health professionals remotely.

The Hypertension Management Service was designed to reduce the need for routine blood pressure readings to be taken in-person. It aims to free up clinical resources and empowers patients to play an active role in their healthcare.

Proof of Concept

We developed the Hypertension Management Service as part of a clinical research project for the Digital Health and Care Institute (DHI).

How it works

Patients use a digital service and blood pressure cuff to collect and share regular blood pressure readings through their smartphone or tablet

Clinical teams can access the patient's electronic health record, view blood pressure readings and securely send messages to patients

Patients receive regular SMS and email reminders to complete readings

Data rules can trigger interventions or escalations

Cost

This service is available on a cost-per-patient basis for a minimum period of 12 months.

Exact costs depend on the number of patients, duration of the trial and integrations with existing systems. This includes patient management systems (PMS) and electronic medical records (EMR).
