Patient gives permissions and shares data from wearables and other connected devices designated by the clinical team
Clinical team accesses full clinical dashboard to efficiently monitor the patient, provide an outcome or implement an action plan depending on the patient's requirements
Specialist remote care is provided to patients at home with data-powered triage for earlier and less costly interventions
